BY THIS DEED, I the undersigned ALICE MANSARAY of Kabawana Section, Kailahun District in the Eastern Province of the Republic of Sierra Leone do hereby:-
- For and on behalf of myself and my relatives wholly renounce, relinquish and abandon the use of my former name of ALICE MANSARAY and in place thereof do assume from the day of April 2021 the name of ALICE SERRAH KUMBA MANSARAY and so that I and my relatives may hereafter be called and refer to me no by my former name of ALICE MANSARAY but my assumed name of ALICE SARRAH KUMBA MANSARAY.
- For the purpose of enhancing such determination declare that I shall at all times hereafter in all Records, Deeds and Writings and in all Proceedings, Dealings and Transactions, private as well as public and upon all occasions whatsoever use and sign the name of ALICE SARRAH KUMBA MANSARAY.
- Expressly authorize and request all persons at all times hereafter to designate and address me by all times hereafter to designate and address me by such assumed name accordingly.
